Vestibular neuronitis, also known as vestibular neuritis, is the acute inflammation of the vestibular nerve. It leads to a sudden onset, severe (peripheral) vertigo, and is commonly associated with a URTI.
It peaks between 40 and 50 years of age.
- Causative organisms
- Herpesviruses (HSV and VZV) are the most common causative organisms
- Patient History
- Relatively healthy patient
- Young or old
- Signs and symptoms
- Vertigo
- Severe and prolonged
- Worsens with head movement
- Nausea and vomiting
- Imbalance
- Nystagmus
- Hearing loss and tinnitus are uncommon since it does not involve the cochlea
- Physical examination
- Positive Romberg’s test
- Abnormal caloric vestibular stimulation test
- Treatment
- Treat dehydration caused by nausea and vomiting
- Antiemetics
- Prochlorperazine to rapidly relieve symptoms in severe cases
- Antihistamines (diphenhydramine, phenergan, cyclizine, or promethazine) in less severe cases
- Anticholinergics or vestibulosuppressants:
- Vestibular rehabilitation exercises for chronic symptoms
