Urge urinary incontinence is characterised by involuntary urine leakage, which is preceded by a perceived, strong, imminent need to urinate.

It differs from Overactive bladder (OAB), which is an umbrella term for increased urinary urgency, with or without incontinence.

Overactive bladder affects men and women equally. However, women and overweight individuals are more likely to be incontinent.

  • Causes (DIAPPERS)
    • Dementia, delirium, or other functional inhibitive handicaps.
      • They are physically or mentally incapable to get to the restroom
    • Infection (UTI)
    • Atrophic vaginitis
    • Psychological
    • Pharmacological
    • Endocrine
    • Restricted mobility
    • Stool impaction or stones (renal calculi)
      • Due to irritation of the detrusor muscles or physical impaction of the urethra)
    • Others
      • Neurological conditions such as stroke, parkinson disease, or multiple sclerosis which can cause detrusor instability
  • Patient history
    • Elderly
    • Post-menopausal
    • History of bladder surgery (possible iatrogenic urethral compression that can cause detrusor instability)
  • Signs and symptoms
    • Urinary urgency
      • Which prompts the patient to need the bathroom enough to interfere with daily activities
      • Waking up to urinate (Nocturia)
    • Urinary incontinence
      • A large amount of urine is lost during an episode
    • Other symptoms
      • Dysuria
      • Increased urinary frequency
  • Physical examination
    • Mostly unremarkable
    • No vagina diverticulae
    • Bulbocavvernous and anocutaneous reflexes are normal indicating an intact S2-S4/pudendal circuit
    • Valsava during the vaginal examination to rule out stress urinary incontinence
    • Q-tip test is negative in pure urge urinary incontinence
  • Investigations
    • Urinalysis
    • Urodynamic studies if a neurologic cause is suspected
      • Involuntary detrusor contractions despite low urinary volume in bladder
  • Treatment
    • Lifestyle modifications
      • Pelvic floor muscle strengthening (Kegel) exercises
      • Avoid caffeine
      • Scheduled urine visits
      • Controlled fluid intake
      • Weight loss
    • Bladder retraining
    • Bladder stabilising drugs
    • Topical estrogens are ****considered in patients with concurrent atrophic vaginitis
    • Surgery
      • Sacral neuromodulation
      • Tibial nerve stimulation
      • Botulinum toxin A bladder injections
      • Augmentation cystoplasty
