Supraventricular Tachycardia (SVT)

Supraventricular tachycardia includes arrhythmias that originate from supraventricular tissue or involve supraventricular tissue through a reentrant circuit.

Definition of terms

Term	Definition
Paroxysmal supraventricular tachycardia (PSVT)	A sudden, intermittent episode of SVT. It may be self-limited – lasting minutes to hours.

Focal Atrial Tachycardia	A rapid pacemaker in the atrial tissue (5%)	Inverted P waves
Multifocal Atrial Tachycardia	Multiple competing pacemaker sites on atria (the most common cause is COPD)	Irregularly irregular rhythm, P waves present with different morphologies
Atrial flutter	A self-sustaining circular electrical movement around the atria	Sawtooth P waves (Atrial rate > Ventricular rate)

Signs and symptoms
- Dyspnea
- Chest pain
- Decreased exercise tolerance
- Anxiety
- Dizziness
- Syncope
Investigations
- Electrocardiogram
  - P waves will be absent or abnormal
- Complete Blood Count
- Electrolytes, including serum Calcium and Magnesium
- Thyroid Panel
- Urine Toxicology screen
- Cardiac enzymes and CXR if there is chest pain
Treatment
- Unstable patient
  - Synchronised cardioversion starting at 100J
- Stable patient
  - Vagal maneuvers, e.g., Carotid massage followed by Adenosine
    - If no response to adenosine, consider that it is sinus tachycardia or atrial tachycardia
- For persistent PSVT
  - Medical prophylaxis with beta-blockers or non-dihydropyridine CCBs
  - Surgical treatment through radiofrequency ablation
Treatment for multifocal atrial tachycardia
- Beta-blockers can terminate episodes
- Manage the underlying condition
