Dysfunctional Uterine Bleeding (DUB) is an outdated term that used to refer to abnormal uterine bleeding that occurs in the absence of recognisable pelvic pathology, systemic disease, or pregnancy. It is typically caused by hormonal imbalances, particularly those involving the hypothalamic-pituitary-ovarian axis, leading to irregular endometrial shedding.

DUB is a diagnosis of exclusion. Most cases of DUB are now classified as AUB-O. Up to 40% of patients initially diagnosed with DUB ultimately have an organic cause identified.

DUB is classified based on the pattern of bleeding and ovulatory status

Pathophysiology of anovulation
- Anovulation is seen in
  - Adolescents (due to immature HPO axis)
  - Perimenopausal women (due to ovarian decline)
  - PCOS (due to hormonal imbalance)
- Lack of ovulation (anovulation) → No progesterone secretion → Unopposed estrogen → Endometrial hyperplasia & irregular shedding.
- This results in irregluar, heavy, or prolonged bleeding and no cyclic menstruation
Pathophysiology of ovulatory dysfunctional uterine bleeding
- Ovulation occurs, but hormonal dysfunction leads to abnormal bleeding.
- Luteal Phase Defect (Shortened Secretory Phase):
  - Caused by stress, hyperprolactinemia or thyroid disorder
  - Inadequate progesterone → premature shedding
  - Bleeding occurs as short cycles (<21 days) and spotting before menses
- Prolonged Secretory Phase
  - Persistent corpus luteum → Prolonged progesterone secretion
  - This leads to prolonged or heavy bleeding
- Midcycle (Ovulatory) Bleeding:
  - Estrogen drop around ovulation → Light spotting
Investigations
- Thyroid function test
- Prolactin
- Progesterone level (on day 21)
- Transvaginal ultrasound
- Endometrial biopsy is > 35 years
