Breech presentation is when the buttocks or feet of the foetus descend into the birth canal first.

It occurs in 3-4% of term pregnancies.

Types of breech presentation

Breech presentationDescription
Breech presentationThe buttocks or sacrum present first, with or without the legs.
Complete breechBreech with the thighs and legs flexed
Frank breechBreech with the thighs flexed and legs extended (feet in face)
Footling breechBreech with the thighs and legs extended. It can be a single or double footling.
  • Risk factors
    • Prematurity is the most common cause
    • Oligohydramnios
    • Multiple gestation
    • Hydrocephalus
    • Placenta previa
    • Contracted pelvis
      • The foetus favours breech presentation in hydrocephalus, placenta previa, and a contracted pelvis since the buttocks are smaller than the head
    • Multiparity
      • Due to a lax abdominal wall
    • Foetal abnormalities
  • Physical examination
    • A broad, soft, irregular, and non-ballotable bulky mass in the presenting area
  • Treatment
    • External cephalic version (ECV) can be attempted from 36 weeks gestation
    • The mode of delivery depends on the gestational age, type of breech, and individual factors
      • Elective caesarean delivery
      • Vaginal breech delivery
  • Complications
    • Cord plrolapse
    • Birth trauma
    • Increased perinatal mortality
