Acute heart failure is a life-threatening emergency that is characterised by the sudden onset or worsening of symptoms of heart failure. It typically presents with symptoms of volume overload.

- Non-compliance or cessation of therapy
- Non-compliance with dietary recommendations
- Uncontrolled hypertension
- Cardiac arrhythmias, especially A-fib and A-flutter
- New or worsening myocardial ischemia or infarction
- Intercurrent infective illness
- Anaemia
- Hyperthyroidism
- Pregnancy
- Pulmonary embolism
- Renal failure
- Worsening underlying heart disease, e.g. further narrowing of a stenotic aortic or mitral valve

- Blood tests to look for anaemia, electrolyte abnormalities, and infection
- Chest X-ray
- Pulmonary venous congestion

- Echocardiogram to identify pericardial effusion and cardiac tamponade
- B-type natriuretic peptide
- 100mg/L indicated myocardial damage
- Treatment
- Upright positioning
- Intravenous loop diuretic (furosemide or bumetanide)
- Oxygen to keep SpO2 at 94 0 98%
- Nitrates if there is concomitant myocardial ischaemia, severe hypertension, or regurgitant aortic or mitral valve lesions
- CPAP for respiratory failure
- For cardiogenic shock (SBP < 85 mmHg):
- Ionotropes (dobutamine)
- vasopressors (norepinephrine)
- Mechanical circulatory assistance (intra-aortic balloon pump or ventricular assist device)
- Morphine may be considered to reduce dyspnoea or distress
- Continue regular medication for heart failure
