Tubulointerstitial nephritis is inflammation of the tubules and interstitial spaces. Drug-induced interstitial nephritis is the most important cause. It can occur at any age and may develop even after prolonged exposure.

Its incidence peaks between the years 60 and 70.

Classification

Causes of tubulointerstitial nephritis

AutoimmuneSLE, Sjogren syndrome, sarcoidosis, and ANCA-associated vasculitides
IdiopathicTubulointerstitial nephritis with uveitis syndrome (TINU), which affects wemon and is associated with HLA-DRB1
  • Pathophysiology
    • Trigger → T cell activation and cytokine production → infiltration of the interstitium → inflammation and oedema → tubular injury which impairs reabsorption → proteinuria, sterile pyuria and eosinophilia
    • Chronic interstitial nephritis → fibrosis
  • Signs and symptoms
    • Fever
    • Rash
      • Maculopapular
      • Non-pruritic
    • Arthralgia
    • Myalgia
    • Lymphadenopathy
    • Hepatosplenomegaly
    • Pleural effusion
    • Encephalopathy and peripheral neuropathy may be rare presentations

    • Complete blood count
      • Anaemia
      • Leukocytosis
      • Eosinophilia
    • Urea and creatinine to assess renal function
    • Urinalysis
      • Haematuria
      • Mild proteinuria
      • Sterile pyuria
      • White cell casts are the hallmark for acute interstitial nephritis
      • Urine eosinophils > 1% of urinary leukocytes
    • Serology if indicated
    • Renal biopsy is the gold standard test
      • Interstitial oedema
      • Infiltration of the interstitium +/- tubules (tubulitis)
      • Granulomas
      • Fibrosis and tubular atrophy in chronic disease
  • Treatment
    • Identify and remove the causative agent
      • Avoid rechallenging the suspected drug
    • Maintain adequate hydration
    • Monitor urine function
    • Corticosteroids if renal function does not improve after removing the offending agent
    • Renal replacement therapy if indicated
    • Chronic intersitial nephritis is treated as chronic kidney disease
