Nephrotoxins are substances that damage the kidney tubules directly or indirectly. Indirect damage may be due to interstitial nephritis.
Examples of nephrotoxins
| Category | Examples | Nota bene |
|---|---|---|
| Analgesics | NSAIDS, aspirin and paracetamol | This is often silent until advanced CKD |
| Antibiotics | Aminoglycosides, co-trimoxazole, penicillins, rifampicin, amphotericin, and acyclovir | Single day doses of aminoglycosides are less nephrotoxic |
| Anticonvulsants | Lamotrigine, valproate, and phenytoin | |
| Anaesthetics | Methoxyflurane and enflurane | |
| Radiocontrast material | Iodinated contrast | AKI develops 48 – 72 hours after intravenous contrast. It is prevented with pre-hydration with IV crystaloids. |
| Proteins | Multiple myeloma, haemolysis, and rhabdomyolysis | |
| Crystals | Urate | |
| Bacteria | Streptococci, Legionella, Brucella, Mycoplasma, Chlamydia, Mycobacterium tuberculosis, Campylobacter, Leptosiprosis, and Syphylis | |
| Viruses | EBV, CMV, HIV, Polyomavirus, Adenovirus, and Measles | |
| Parasites | Toxoplasma and Leishmania | |
| Others | PPIs, cimetidine, furosemide, thiazides, ACE inhibitors, ARBs, lithium, iron, calcineurin inhibitors, cistplatin, ethylene glycol, radiation, and aristolochic acid |
