Mullerian hypoplasia, AKA Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome, occurs when both Mullerian ducts fail to form. Complete failure to form is known as Mullerian agenesis, while partial development is known as Mullerian hypoplasia. It presents as primary amenorrhoea since the proximal vagina, cervix, uterus, and fallopian tubes are absent.
Mullerian agenesis accounts for 15% of cases of Mullerian abnormalities. 1/3 are associated with renal anomalies, while 10% have cardiac anomalies
- Signs and symptoms
- Primary amenorrhoea in the presence of thelarche and adrenarche (ovaries are presenta and the patient is hormonally normal)
- Investigations
- Pelvic ultrasound
- Pelvic MRI
- IV pyelography and echocardiography to rule out renal anomalies
- Echocardiography to rule out cardiac anomalies
