Inflammatory bowel disease (IBD) includes Crohn’s disease and ulcerative colitis. They are characterised by relapsing and remitting inflammation of the gastrointestinal tract.

Crohn’s disease vs Ulcerative colitis

Crohn’s diseaseUlcerative colitis
SymptomsNon-bloody diarrhoea, more prominent weight loss, upper gastrointestinal symptoms, mouth ulcers, and perianal diseaseBloody diarrhoea, tenesmus, and LLQ abdominal pain
Extra-intestinalGallstones and oxalate renal stonesPSC
ComplicationsBowel obstruction, fistula, and colorectal cancerThe risk of colorectal cancer is higher
HistologyTransmural inflammation, increased goblet cells, and granulomasMucosal inflammation, depleted goblet cells, and infrequent granulomas
EndoscopyDeep ulcers with skip lesions, and ‘cobble-stone’ appearanceWidespread continuous ulceration with pseudopolyps
RadiologyStrictures (Kantor’s string sign), proximal bowel dilation, ‘Rose thorn’ ulcers, and fistulaeLoss of haustrations, superficial ulcerstion, pseudopoplyps, and ‘drainpipe colon’ (narrow and short colon)

Extra-intestinal manifestations of IBD

CategoryExtra-intestinal manifestationsNota bene
Related to disease activityArthritis (asymmetric and pauciarticular), erythema nodosum, episcleritis, and osteoporosisArthritis is the most common extra-intestinal manifestation. Episcleritis is more common in Crohn’s disease
Unrelated to disease activityArthritis (symmetric and polyarticular), uveitis, pyoderma gangrenosum, clubbng, primary sclerosing cholangitis (PSC)PSC and uveitis are more common in ulcerative colitis.
