Hypertensive emergency, also known as symptomatic essential hypertension, is defined as hypertension with organ-specific symptoms. The blood pressure is usually >180/110 mmHg. It is characterised by hypertension (x1) with neurological, cardiac, or renal symptoms.

Definition of terms

Malignant hypertensionAn archaic term used to refer to a hypertensive emergency accompanied by fundoscopic changes (raised intracranial pressure). This requires a CT scan to rule out intracranial haemorrhage.

Clinical features of hypertensive emergency

  • Treatment of a hypertensive emergency
    • According to hospital guidelines
    • Establish IV lines
    • Crash box ready since V-fib can occur
    • Set up the cardiac monitor
    • Intravenous antihypertensives (hydralazine or labetalol)
      • Keep diastolic BP at 100-130 mmHg
      • Decrease MAP by 25% within 2-6 hours of admission, and to achieve normal MAP by 24 hours.
      • A rapid drop in blood pressure should be avoided since it can cause a stroke
    • Switch to multiple oral antihypertensive therapy following BP control
