Bypass urinary incontinence occurs when urine leaks after avoiding the normal continence mechanisms. It is usually due to fistulas, which become apparent between 1 and 2 weeks after the inciting event. The risk for developing fistulas following surgery is higher in women with pre-existing pelvic pathology e.g. endometriosis, pelvic inflammatory disease, and pre-operative radiation for malignancy

Causes
- Obstetric trauma following complicated labor is the most common cause in developing countries
  - Obstructed labour
  - Prolonged second stage of labour
  - Operative delivery
- Pelvic surgery is the most common cause in developed countries
  - Hysterectomy
- Pelvic radiation
Signs and symptoms
- The incontinence is similar to overflow urinary incontinence
  - Constant slow loss of urine
- However, the patient will be able to void when desired
Investigations
- Urinalysis and culture to rule out concurrent UTI
- Double dye test, where the vagina is packed with sterile gauze and methylene blue dye is introduced intravesically via a foley catheter, and phenazopyridine (pyridium) is given orally
  - Instillation of methylene blue into the bladder identifies vesicovaginal fistula. Methyleneblue crosses from the bladder and into the vagina staining the gauze blue
  - Oral phenazopyridine (pyridium) identifies ureterovaginal fistula. Pyridium crosses the uterus into the vagina staining the tampon orange

- IV pyelography
- Cystoscopy
- Retrograde pyelography is the most definitive test to diagnose a ureterovaginal fistula
