Acute pericarditis is inflammation of the pericardial sac for a period of less than 4 – 6 weeks. It is the most common pericardial disease.

It peaks between 40 and 50 years of age.

Causes of pericarditis

  • Pathophysiology
    • Inflammation → increased vascular permeability
      • Pericardial effusion → cardiac tamponade if severe
      • Fibrin deposition → fibrinous pericarditis → constrictive pericarditis
  • Signs and symptoms
    • Chest pain
      • Retrosternal
      • Sharp or pleuritic
      • Improved by sitting up and leaning forward
      • Radiates to the trapezius ridge (specific)
    • Pericardial friction rub
      • Superficial, scratchy, or squeaky quality on auscultation using the diaphragm
      • Loudest at the left lower sternal border
      • Differentiated from pleural rub by asking the patient to hold their breath
    • Low-grade fever
    • Myalgia and malaise

  • Investigations
    • Electrocardiography
      • Widespread concave ST-elevations with PR-segment depression
      • T-wave changes
    • Raised CRP, ESR, or WBC
    • Raised troponins in case of myopericarditis
    • Elevated urea in uraemia
    • Cardiac MRI to confirm myocardial involvement
    • Pericardiocentesis in case of bacterial or neoplastic causes
    • Blood culture if fever > 38 C
    • HIV serology
    • Interferon-gamma release assay or tuberculin skin test
  • Treatment
    • Inpatient treatment for patients with fever > 38 C and elevated troponins
    • Treat the underlying cause
    • Avoid strenuous physical activity
    • NSAIDs and colchicine are first-line treatments for acute idiopathic or viral pericarditis
  • Complications
    • Pericardial effusion
    • Cardiac tamponade
    • Constrictive pericarditis
    • Recurrent pericarditis
