A 40-year-old with wrist pain

Last updated: June 4, 2026Bookmark

A 40-year-old male presents after being struck on the back of his wrist by a heavy metal object at work. He has localised pain on the radial side of his wrist, limited wrist movement and swelling.

Oblique distal radius fracture with intra-articular extension
Q1. What is the name of this type of fracture, and which bone is involved?
  1. Chauffeur or Hutchinson fracture
  2. Oblique radius styloid fracture with intra-articular extension
Q2. What is the classical mechanism of injury of this fracture?
  1. Fall onto outstretched hand
  2. High-mechanism injury e.g. road traffic accident
Q3. Which carpal bone typically causes this fracture by compression?

Compression of the scaphoid against the styloid process of the radius

Q4. What associated injuries must be ruled out?
  1. Scapholunate dissociation: this is especially true when the fracture line involves the articular surface near the scapholunate interval
  2. Trans-caphoid perilunate dislocation
  3. Unlar-styloid fracture (Frykman type IV)
Q5. What are some of the indications for the surgical treatment of this type of fracture?
  1. Open fracture
  2. Displaced intra-articular fracture > 2mm
  3. Highly comminuted fracture
