Croup, also known as laryngotracheobronchitis, is an inflammation of the tracheobronchial airway. It has a more severe presentation, particularly in young infants who have a narrower airway.
Croup accounts for up to 15% of pediatric URTI Emergency Department visits. It peaks between 6 months and 3 years of age, and affects boys slightly more than girls.

Inspiratory stridor following URTI
Signs and Symptoms
Sudden onset of hoarseness and “seal-like” barking cough
Low-grade fever
Inspiratory or biphasic stridor
Croup can involve the upper trachea
Variable levels of respiratory distress
Cyanosis
Suprasternal recession
Intercostal recession
Subcostal recession
Nasal flaring
Lethargy
Respiratory arrest

Spasmodic (recurrent) croup

Complete Blood Count: to rule out differentials caused by bacterial infection, e.g., tracheitis, epiglottitis, and retropharyngeal abscess
AP and lateral neck radiograph:
Steeple sign on AP – this is almost pathognomonic for Croup. It is also seen in bacterial tracheitis
PCR for RSV, parainfluenza virus, and other viruses
Treatment
Assess severity using the Westley croup score
Monitor pulse oximetry, heart rate, respiratory rate, and level of consciousness
