Anticoagulation therapy prevents and treats thromboembolism by inhibiting the coagulation cascade.
Anticoagulants
| Class | Mechanism | Examples | Nota bene |
|---|---|---|---|
| Unfractionated heparin (UFH) | Enhances the activity of antithrombin III → decreased thrombin and Xa activity | Heparin | Monitored with aPTT and Anti-Xa levels. It is reversed with protamine sulphate |
| Low molecular weight heparin ( LMWH) | Inhibits Xa | Enoxaparin and dalteparin | Monitored with anti Xa levels in pregnancy and renal failure |
| Vitamin K Antagonists (VKA) | Reduces the activity of Factors II, VII, IX, and X | Warfarin | Monitored with PT or INR. It is reversed with Vitamin K, 4F-PCC, and FFP |
| Direct thrombin inhibitors | Directly inhibits thrombin (IIa) → reduced fibrin | Dabigatran, argatroban, and bivalirudin | Dabigatran is reversed with idarucizumab |
| Direct oral anticoagulants (DOACs) | Inhibit factor Xa | Apixaban, rivaroxaban, edoxaba, and betrixaban | They have fixed dosing and require less monitoring. They can also be reversed with adexanet alfa. |
Conditions and their preferred anticoagulants
| Condition | Preferred Anticoagulant | Nota bene |
|---|---|---|
| Nonvalvular atrial fibrillation | Apixaban or other DOACs | DOACs preferred over warfarin in most patients |
| Mechanical heart valve | Warfarin | DOACs contraindicated |
| Deep vein thrombosis (DVT) | Rivaroxaban or Apixaban | DOACs commonly first-line |
| Pulmonary embolism (PE) | Apixaban or Rivaroxaban | Heparin often used initially if unstable |
| Acute coronary syndrome / PCI | Heparin | UFH preferred peri-procedurally |
| Immediate anticoagulation needed | Heparin | Rapid onset and reversible |
| Pregnancy | Enoxaparin | Warfarin teratogenic in first trimester |
| Cancer-associated thrombosis | Enoxaparin or DOACs | LMWH traditionally preferred |
| Heparin-induced thrombocytopenia (HIT) | Argatroban | Avoid all heparin products |
| Renal failure | Heparin | Safer because not renally cleared |
| Cardioversion for atrial fibrillation | Apixaban or Warfarin | Anticoagulation required before/after cardioversion |
| Left ventricular thrombus | Warfarin | INR target usually 2–3 |
| Prosthetic heart valve | Warfarin | Lifelong anticoagulation |
| Perioperative bridging | Enoxaparin | Used when stopping warfarin temporarily |
| Disseminated intravascular coagulation (selected chronic thrombotic cases) | Heparin | Used cautiously |
| Severe obesity or antiphospholipid syndrome | Warfarin | More evidence than DOACs in these settings |
Monitoring tests
| Test | Purpose |
|---|---|
| PT/INR | Warfarin monitoring |
| aPTT | Heparin monitoring |
| Anti-Xa | LMWH/DOAC monitoring |
| CBC | Platelets/anemia |
| D-dimer | Fibrinolysis marker |
| Thrombin time | Thrombin inhibition |
