Statins inhibit the action of HMG-CoA reductase, which is the rate-limiting enzyme in hepatic cholesterol synthesis. They are usually taken at night since this is when the majority of cholesterol synthesis takes place.

The goal of treatment with statins is to reduced LDL by ≥ 50%.

  • Indications
    • Primary prevention (low-dose statin therapy)
      • 10 year cardiovascular risk ≥ 10%
      • Type 1 diabetes for > 10 years, or > 40 years old, or established nephropathy
      • Type 2 diabetes (assess using QRISK2)
      • CKD with eGFR <60ml/min/m2
    • Secondary prevention (high-dose statin therapy)
      • Ischemic heart disease
      • Cerebrovascular disease
      • Peripheral arterial disease
  • Adverse effects of statins
    • Myopathy: more common with lipophilic statins (simvastatin, artovastatin)
      • Myalgia
      • Myositis
      • Rhabdomyolysis
      • Raised creatine kinase levels
    • Liver impairment:
      • Monitored with LFTs at baseline, 3 months and 12 months.
      • Treatment should be discontinued if transaminases are raised and persist 3 times upper limit of normal
    • Intracerebral hemorrhage:
      • Due to this, statins are not recommended for secondary prevention in patients with intracerebral haemorrhage (haemorrhagic stroke)
  • Risk factors for statin induced myopathy
    • Advanced age
    • Female sex
    • Low BMI
    • Presence of multisystem disease, e.g., diabetes
  • Contraindications
    • Pregnancy and lactation
    • Active liver disease
    • Macrolide antibiotics (erythromycin and clarithromycin)
      • Statins should be discontinued until the course of antibiotics is completed
