Group B streptococcus (GBS) is acquired from a mother who is carrying GBS in her vaginal flora, without circulating anti-GBS IgG. The neonate will not have passive immunity against GBS once they are exposed in the birth canal.
The sequelae of infection can occur early ( in less than 7 days) or later (from 7 days to 4 months).
- Risk factors for GBS infection.
- Prematurity – gestational age <37 weeks
- Rupture of membranes> 18 hours
- Young maternal age
- Obstetric history of a previous birth with GBS infection
- Signs and symptoms of early-onset neonatal GBS infection (less than 7 days)
- Onset typically within 48 hrs of birth
- Apnea
- Ill-appearing
- Irritable
- Temperature instability
- Signs consistent with shock
- Signs and symptoms of Late-onset neonatal GBS infection (7 days to 4 months)
- Ill-appearing
- Meningitis
- Pneumonia
- Sepsis
- Other focal infections (eg, cellulitis, acute otitis media, and conjunctivitis)
- Investigations
- Chest radiograph:
- Diffuse, homogenous, ground-glass infiltrates
- May look like NRDS
- Complete blood count
- Leukopenia with left shift
- Blood and CSF culture

- Broad-spectrum antibiotics
- Benzyl-penicillin, and either gentamicin or cefotaxime
- Cefotaxime is preferred over gentamicin when the spinal tap is suggestive of meningitis

- Rectal and vaginal swabs for GBS culture are done for all women at 35-37 weeks of gestation
- Intrapartum prophylaxis
- Intravenous Penicillin G or ampicillin
- Intravenous cefazolin is given instead if there is a mild penicillin reaction
- Intravenous clindamycin is given if there is a severe penicillin allergy
- Screening
- Indications for intrapartum prophylaxis
- Maternal GBS colonization – Positive Rectal or vaginal swabs at 36 0/7 – 37 6/7 weeks
- GBS bacteriuria during pregnancy
- The previous newborn had a GBS infection
- Presence of risk factors
- Chorioamnionitis
- Fever
- Elevated CRP
- Premature contractions
- PROM
