Adrenal Crisis (Addisonian Crisis)

Adrenal crisis is an acute life-threatening condition characterised by severe hypotension, hypoglycaemia, and altered mental status. It can occur in patients with known Addison’s disease when oral steroids are not increased to cover stress or when dosages have been missed. It can also be caused by bilateral adrenal haemorrhage, e.g., in meningococcaemia

  • Signs and symptoms
    • Shock
    • Altered mental status
  • Treatment
    • Intravenous fluid resuscitation
    • Intravenous or intramuscular hydrocortisone
    • Correct hypoglycaemia
    • Monitor glucose, electrolytes, and fluid balance
    • Blood for cortisol and ACTH
    • Blood, urine, and sputum for culture if there is concern for infection
    • Oral steroids +/- fludrocortisone once the patient is in good condition
