A 45-year-old woman with breast cancer on adjuvant chemotherapy presents with a fever of 38.9°C, chills, and malaise. She looks acutely unwell. On physical examination, she has hypotension (BP 90/60 mmHg), tachycardia , with no localising source of infection.
Her laboratory results: WBC 0.3 ×10⁹/L, neutrophils 0.1 ×10⁹/L.
Q1. What is the diagnosis?
The diagnosis is neutropenic fever or sepsis
Fever is defined as a single oral temperature of≥ 38.3 C or an oral temperature of ≥ 38 C sustained for 1 hour
Severe neutropenia is defined as ANC < 500 or expected to drop to < 500 within 48 hours
Neutropenia tends to occur 7 days after cytotoxic chemotherapy and lasts ≤ 7 days in patients with solid tumours.
Leukaemia/lymphoma regimens may cause longer-lasting, profound neutropenia
Mean time to defervescence is 5 days in haematologic malignancies and 2 days for solid tumours

There is a trend towards Gram-positive organisms due to the introduction of long-term indwelling catheters
Q4. What are the risk factors for developing neutropenic fever?
Older age
Poor performance status
Haematologic malignancy
Medical comorbidities, e.g., chronic kidney disease
Low body mass index
Low baseline WBC before chemotherapy
Q5. What is the pathogenesis of neutropenia fever?
Reduced WBC production → reduced phagocytosis, reduced production of inflammatory mediators
Possible sources of infection: disrupted gastrointestinal mucosal barrier, indwelling catheters, altered gut microbiome
Q6. What is the initial management of this patient?
Broad-spectrum IV antibiotics immediately (within 1 hour)
piperacillin/tazobactam or cefepime or meropenem or imipenem
Add aminoglycoside or fluoroquinolone + vancomycin if shock/severe sepsis
Add vancomycin or linezolid if evidence of skin/soft tissue infection
Amoxicillin/clavulanate can be given for low-risk patients
Avoid digital rectal exam or manipulation
Careful oral examination and examination of catheter sites, if any
Investigations to order: Blood culture (preferably before IV antibiotics, but should not delay the antibiotics)
Urinalysis and urine culture
Lumbar puncture, abdominal CT, or echocardiogram is recommended if symptoms suggest infection in the relevant location
A central venous catheter can be removed if there is a proven line infection, endocarditis , or S.aureus/pseudomonas/fungal infection.
G-CSF, e.g., filgastrim, pegfilgastrim, is used for prophylaxis
The duration of antibiotics is until the patient is afebrile for > 2 days or ANC > 500 if the source is unknown.
If the source is known, antibiotics are given for the usual duration of the source, and until ANC reaches > 500
Antifungals, e.g., liposomal amphotericin B, voriconazole, and caspofungin, are added after 5 – 7 days with persistent fever without a clear source
